If your child is under 2, we need all the forms listed above. 
If your child is over 2, we need all forms except: Infant feeding schedule, pediatric health statement

If your child does not take regular medications, we still ask that the Medication Authorization form be filled out for Tylenol or Ibuprofen and Benadryl JUST IN CASE we need to use them. We never give medication without parental permission at the time it is to be used, but we cannot give the medication without the form, even with written parent permission otherwise.
